Eligibility for Services

K&B Connect LLC serves individuals who qualify under Maryland Medicaid programs, including the Community Options Waiver (CO Waiver).

Who Qualifies

Basic Eligibility Criteria

To qualify for residential services through the Community Options Waiver, individuals must generally meet the following criteria:

  • 🔞
    Age 18 or older

    Adults 18 years of age or older residing in Maryland.

  • 🏥
    Nursing Home Level of Care

    Individual requires a level of care equivalent to skilled nursing facility placement.

  • 💳
    Medicaid Eligible

    Must have current Medicaid eligibility or be in the process of applying.

  • 🩺
    Medical or Behavioral Needs

    Has documented medical, physical, or behavioral health needs requiring residential support.

  • 🤲
    ADL Assistance Required

    Requires assistance with one or more Activities of Daily Living (ADLs).

  • 🏠
    Supervised Housing Required

    Benefits from or requires a structured, supervised residential environment.

How to Apply for the CO Waiver

Reach Maryland's No Wrong Door system — the starting point for all waiver applications in Maryland.

Complete Eligibility Assessment

A state assessor will conduct a medical and financial eligibility evaluation to determine your level of care needs.

Work with a Client Support Planner

A CSP (Client Support Planner) will be assigned to help you navigate service options and select providers.

Select K&B Connect LLC

Choose K&B Connect LLC as your Residential Service Agency provider when completing your service plan.

Assessment & Placement

Our team conducts an intake assessment and coordinates a residential placement that meets your needs.
